Dixie came through. She runs the PF Museum (and is the curator), and serves as the NAD's PF historian. Here's what she wrote:
This patch was used in the Canadian Union (now SDA Church of Canada) 1960's until late 1970's on the Uniform cap... We have one for the display but I do not know who designed it originally.
