Usually called an Unknown cache and is displayed on the map with a ?. These can be puzzle caches where you may have to spend lots of time deciphering a code or puzzle before you go to find the container. The container usually is NOT at the listed coordinates. You can have lots of fun and frustration solving these. You can also message others who have found it or even the CO for some further hints. On puzzle caches (Mystery caches) once you have solved the puzzle you can choose to or what ever the puzzles is it may change to the place of the ? on the map to the final spot displayed as a jig saw puzzle piece. To do it manually on the cache page on the right side of the coordinates there is an edit mark and you can change the coordinate to the real container site. Once you log the find after you search you will notice the smillie goes back to the original? spot on the map. There are many Geo Art "pictures" with lots of ? forming shapes. It is a good result to complete the shape.
